I was born in Brackley Pt, Prince Edward Island on the 30th day of May anno Domini 1898; I now reside at 6 Blind Bridge St. Winchester, Massachusetts. I emigrated to the United States of America from said Brackley Pt on the vessel via rail; my last foreign residence was said Brackley Pt; I am not married; ... It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign prince, ... particularly to George V King of Great Britain and Ireland, of whom I am now a subject; I arrived at the port of Vanceboro, in the State of Maine, on or about the 20th day of November, anno Domini 1923; .... it is my intention to become a citizen of the United States of America and to permanently reside therein; So Help Me God.
